Our Critical Care Unit is a mixed medical and surgical ICU with 17 beds. We also provide assistance in the postoperative care unit (25 beds), trauma patients and in-hospital cardiac arrest.

Techniques You Will Learn

Techniques you will learn during your program here:

The aim of this program is to give the fellow a comprehensive exposure to clinical respiratory management in critical care patients including inpatient and outpatient diagnosis and management. The fellowship is designed to flexibly accommodate the varying educational needs of the trainee and the duration of the program.

The program covers the fundamentals and clinical skills in patho-physiology, monitoring and application of respiratory assistance.

Curriculum

Training Program Includes:
Fundamentals in pulmonary physiology: respiratory mechanics, gas exchange, respiratory mechanics…
Fundamentals of the most frequent pathophysiological disorders in critical care patients: acute respiratory failure, acute respiratory distress syndrome, acute lung edema, COPD, pneumonia….
Fundamentals and clinical skills of the different non-invasive ventilatory techniques: high flow therapy, continuous positive airway pressure, bilevel positive airway pressure, pressure support.
Fundamentals and clinical skills in non-invasive ventilation
Fundamentals and clinical skills in invasive mechanical ventilation
Fundamentals in procedures such as alveolar recruitment maneuvers or PEEP adjustment.
Introduction to the different monitoring tools such as lung ultrasonography, volumetric capnography, transpulmonary pressure or electrical impedance tomography.
Fundamentals and clinical skills in weaning from mechanical ventilation

Activities:

Fellows will be given the opportunity to take part of the dynamics of the unit taking part in the rounds, clinical discussion and management of all those patients admitted to our ICU that require some type of ventilatory support. Fellows will participate in clinical training sessions in respiratory physiology and mechanical ventilation, as well as in the clinical sessions of the ICU and the anesthesiology department. They will also be encouraged to present their work orally at a group meeting at least once before they complete their visit. Fellows will be given the opportunity to participate in both clinical and experimental studies carried out during the stay. At the end of training the fellow will receive a certificate attesting training attendance.

The host mentor provided me lessions on mechanical ventilation and together we made rounds of patients with the rest of ICU team. Also he was available for all my needs, easy to communicate and helpful in organization of my fellowship. The Fellowship provided me a new experience on management of criticaly ill patients, I learned different approach in treatment of patients with septic shock, use of bed-side ultrasound and improved my skills for mechanical ventilation. I also saw how the working process in the ICU is organized that might be implemented in my Department to improve its management. The negative aspect of the program is that all the work in the department was on Spanish language that I don`t speak, the staff spoke on spanish all the time while planning the patients treatment and during rounds, so it was a bit difficult to follow and join the team during the work.
